The Medical Board of California has reviewed the qualifications of the physician named
hereon, and has determined he is eligible for appointment as an
EXPERT MEDICAL REVIEWER.

is hereby certified for a two-year period beginning August 1, 2005
to provide expert reviews at the request of the Board,
and to be compensated for services provided.
